WHAT TO BRING TO YOUR FIRST APPOINTMENT

If you have dental insurance, please bring your insurance information to your visit. When you arrive, check in at the reception desk and present your insurance information to the receptionist.


WHAT TO EXPECT

Your first visit to Dental Faculty Associates will require extra time in order to complete your medical history and other forms. Please arrive 15 minutes before your scheduled appointment. Patients under 18 years of age must be accompanied by a parent or guardian for their first visit.
