Accepting The Offer Of Admission

To accept the offer of admission, students must take the following first steps.

  1. $1,500 enrollment deposit - Students must pay a one-time enrollment deposit of $1,500 (applied to tuition expenses) online through Wolverine Access. Detailed instructions will be provided with the offer of admission. This deposit is non-refundable.
  2. Criminal background check - Shortly after being offered admission, students will receive an email from Certiphi Screening Inc. with instructions on how to initiate the criminal background check process. All admitted students will be required to complete this background check.
  3. UM Campus ID (uniqname) - After paying the enrollment deposit, students should create and set up their University of Michigan email username and password. Instructions will be sent via email and can also be found at Uniqname & Account Setup.

Matriculation Documents

Admitted students must complete and turn in the following documents to finalize admission. Detailed instructions and links to the forms will be provided upon receipt of the enrollment deposit.

International Student Supplemental Application Form (ISSA)Students requesting a new visa or visa transfer will need to submit the ISSA Form to [email protected] to initiate this process. Detailed instructions on who should submit this form will be sent via email.
Personal Data SurveyIncoming students are required to fill out a secure online survey to provide the School of Dentistry with additional information.
Personal Health ResponsibilitiesStudents must read and electronically sign an online document explaining the personal health responsibilities of a dental student (immunizations, health insurance, etc.).
Technical StandardsAll dental students must certify that they possess certain abilities integral to the dental profession, as outlined in the Technical Standards document. A specific link to electronically sign this document will be emailed.
Record of Required ImmunizationsStudents are required to demonstrate immunity to certain diseases by having a health care provider complete and sign the Record of Required Immunizations.

Official Documents

To finalize admission, students must submit National Board score reports, final dental school transcripts, and notarized copies of dental degrees/diplomas to the Office of Admissions. Instructions will be sent via email upon receipt of the enrollment deposit.

NBDE scoresScore reports should be sent directly from the ADA by email to [email protected]. Scanned copies or photos of reports are not accepted.
TranscriptsStudents must have their official transcripts, with final grades for all coursework, sent directly from all dental schools attended to the Office of Admissions.
Notarized degree/diplomaStudents must submit a notarized copy of their dental degree.

Health Insurance

All dental students are required to demonstrate a minimum level of health insurance coverage. Students will attest to this coverage online through Wolverine Access after the start of the program. See Mandatory Health Insurance Guidelines for more information.

Social Media Guidelines

Representing the University of Michigan and the School of Dentistry is an honor and a privilege
provided to a select group of individuals. Along with that privilege comes a set of expectations
and responsibilities as a Michigan student.


Through social media, you are being monitored by more individuals than ever before.
Everything you do in these forums should positively represent the University, the School of
Dentistry and your profession. Used responsibly, social media can be a great way to interact
with friends, colleagues, prospective students and the public and promote the School and the
profession of dentistry. Used irresponsibly it can be a quick way to destroy your reputation in
140 characters or less…and to potentially put you at risk for dismissal! This resource provides
some tips and suggestions for using social media responsibly and effectively.


• DO set your security settings so that only your friends can see your account.
• DON'T accept friend or follow requests if you are not sure who they are coming from.
• DO understand that who you have listed as Followers or Friends is a reflection on you.
• DON'T put anything on social media that you would not want your family, your future
employers, those reading the front page of the paper, or the whole world to see.
• DO think before you post, tweet or retweet – “Will this positively reinforce my image and
my reputation”?
• DON’T post patient photos or personal patient information.
• DO remember that sharing patient information is a HIPAA violation and is subject to
sanctions.
• DON'T post offensive language, personal attacks or racial comments.
• DO ask questions if you are not sure what you are doing is ok.
• DON'T post or tweet material presented during a course.
• DO talk to the Director of Communications about how to use social media to your
advantage.
• DON'T post when you are emotional. You are more likely to say something you will
regret.
• DO be familiar with the University’s social media policy (SPG 601.07) and the
consequences for violating it.
• DON'T post anything that could be construed as an endorsement or promotion of a
business product or service, even if the business is owned by a relative or friend.
